Immunizations and Physicals

New Students: All new students must have a complete Florida School Entry Health Exam(DH3040) and must be immunized (DH680) according to Florida State law. Students may not start school until these requirements have been met. The following are the two forms that you must have your Florida physician fill out and they must be submitted with your application. Applications cannot be moved forward without these items.

1. DH3040 Florida School Entry Health Exam: must be dated within the previous 12 months from the entry date into school. (this is different from a sports physical. Sports physicals cannot be used for school entry)
2. DH680 Florida Certification of Immunization form: the Diocese of St. Petersburg does not recognize religious exemptions for immunizations.

New and Current Students: If you are entering 7 or 8 grade, there is an additional immunization required. The name of it is Tdap.
